Anaesthesia: local anaesthetic, with sedation if you prefer.

Duration: about 30 to 45 minutes.

Incision: a small cut inside each cheek, so there are no visible scars.

Technique: a carefully measured portion of the fat pad is removed. We favour a conservative approach, keeping enough fat to protect your face as it ages.

Stitches: dissolvable, so nothing needs to be removed.

With rhinoplasty: a refined nose can make full lower cheeks more noticeable. Slimming the cheeks at the same time helps the whole face look balanced, not just the nose.

With double chin liposuction: treating the cheeks and the area under the chin together refines the lower face from both sides, giving a cleaner line from cheekbone to jaw.

With jawline or chin filler: reducing fullness in the cheeks and adding definition to the jaw work well together. Filler is usually done once the cheek swelling has settled.

With a facelift or mini facelift: in some patients, buccal fat that has dropped with age adds to the heaviness of the jowls. Your surgeon may address it during the lift. Because the face loses volume with age, this is decided very carefully and only a small amount, if any, is removed.

Days 1–3: swelling and tenderness in the cheeks. Your face may look fuller than before at first.

Week 1: soft food only, and rinse your mouth as advised to keep the incisions clean.

Weeks 2–3: most swelling has gone and most people are back to normal life.

Months 2–3: the final shape of the cheeks becomes visible.

Over-removal: the most important long-term risk. Too much fat removed can make the face look hollow or older over time. This is why we remove fat conservatively.

Asymmetry: one side can heal slightly differently from the other.

Swelling, bruising and infection: usually minor; good mouth hygiene reduces the risk of infection.

Injury to the salivary duct or a branch of the facial nerve: rare, because these structures are close to the fat pad. Careful technique keeps the risk low.
